Community Service/Volunteer Hours Forms
Making our communities better places in which to live is another core value put into action by American Legion Auxiliary members every day across the country.
When you volunteer, you join other like-minded people who deeply care about our country. The volunteer opportunities are flexible, so you do what you can when you can. And, along the way, you just might make some new friends.

The improved online form is the fastest & easiest way to report your hours. The form now allows you to
Report up to FIVE different volunteer activities on one form
No need to remember the exact day you volunteered — just enter the month and year.
Combine similar activities. For example, if you worked Bingo twice during the same month, simply total your hours and report them as one activity.
The Original form is available for printing.
The Fillable form can be filled out on your computer and emailed to goldstarunit191@gmail.com. It can also be printed either as a blank form or as a completed form.
The Online form will allow you complete a form that will send your hours automatically to Unit's Community Service Chair.
